Koi Angelfish Sac

In my 35 gallon Hexagon tank i have 3 Koi Angelfish. One of them has a round sac protruding in between its ventral and anal fin. It seems to have grown and disappeared several times, but this time it is especially big.

Does it have to do with mating and/or is it something I have to worry about?

I bought all 3 fish at the same time and have had them for about 3 years. The one with the sac is the smallest one of the 3 and I do not know what the sex of any of them are.

I don't know anything about these fish but it looks like a typical egg bound female.
Some people have had success treating african cichlids with Epsom salts but like I say, I don't know anything about these fish and not sure if that would be a wise thing to do.
I have only ever had fish die as a result of being egg bound personally.
