Hole in the head disease

I have a 9 year old black striped severin. I noticed hole in the head for a couple of years now. The fish eats well and is active. I am wondering if I should do nothing or treat the fish with API General Cure (metronidazole/praziquantel, 250/75 mg). I am concerned of toxicity since the fish is old. I am dolng my best to keep the tank clean. It is possible that once in a while I should have been more diligent on tank maintenance but now I am paying better attention. Again the fish is active and eats well. What do you all think?

David, sorry no one has responded.
I've had excellent success with Tetra Parasite Guard. It was recommended to me by a friend who runs a Trout Farm. A very unappreciated name brand. And very inexpensive. Cleared up HITH with my Polleni in 4 days. He hardly has a scar now. And it didn't hurt anyone else in the tank, and I have a variety.
